Missed appointments:
There is a $25 missed appt. fee/$50 for a missed physical. We require at least 24 hour notice to cancel without charge.
Additional Fees:
There is an additional $ 10.00 dollar fee for completion of forms for use outside of the patient's medical record, including:
Phone Care:
There is a $25 fee for suitable "visits" that occur via phone or email. If your question requires a fee or would be suitable for a phone visit, the staff will offer this option to you before any charges are made.
